The template should be printed on heavy cardstock US Letter or A4 Paper sizes. See paper size comparison for more info on A4 size paper. Check printer settings in the instructions below.
Instructions

Download the personalized template PDF above. Click file to unzip. Right click on PDF to Open With > Adobe Reader.
Click and highlight the wording in the PDF form fields to personalize text. There is an extra line of text at the bottom for a total of 4 fields and 2 on the sides.

In Adobe Reader, select File > Print (input page number or current page to print page 2 only) and make sure Page Scaling is set to NONE.
Select quality and paper settings to print on cardstock or heavy paper at best quality.

Use the scissors to cut out around the ticket first to make it easier for detailed cutting.

Cut slowly and carefully around the outside of the ticket.
Turn the paper into the scissors to cut the rounded edges.

Cut around the small perforations to make the ticket look real.

Complete.
You can print and cut more than one to present to your loved one.
