Jukebox CD Holder
A custom jukebox cd holder papercraft to creatively present your mix cd full of songs representing precious moments in your relationship. One of the most popular ways of expressing your feelings is through a mix of music made personal by the choice of songs and how they are arranged in a playlist.
There is no need to limit the mix to just love songs. Any song that has meaning or significance for the both of you will be perfect. Maybe there was a song you sang together while driving. A private joke or reference to something you've laughed about can also add some fun to your playlist.
The jukebox stands for a unique presentation or folds flat for mailing if you have a long distance relationship. A cd slides in on the back and is held securely with a tab.
The text can be personalized by filling in the form fields of name, message, and from as well as an optional loving message on the back of the jukebox cd holder.

Download the personalized template PDF above.
Click and highlight the wording in the PDF form fields to personalize with the name of your sweetheart and your own message.
Repeat the same way for the back or click and highlight text to delete.
Click the print button to the right and select Current page (page 2 only) before printing.
Set paper quality/type to best on card stock paper before printing and load printer tray with card stock paper.
Repeat to print page 3 only.
Use the paper clip and ruler to score the fold lines on the back. The fold line guides are the blue dashed lines.
The curved score line will need to be done the same way by slowly following the curved line with the paper clip while pressing down to indent the paper.
Once all fold lines are scored, use the craft knife and ruler to cut out the front and back around the outside edges. When cutting, keep the cut path on the line so that you do not see white around the edges.
You can also use a scissor.
Use the craft knife to cut the half circle tab line.
Cut on the blue line only.
The result of the cutouts are as shown.
Fold back along the top of wood grain down on score line.
Turn over and place double sided tape or glue on either side of the half circle tab directly under the fold line as shown.
Do not place any tape or glue directly under the half circle tab.
Fold side tabs up.
Press firmly on each side.
Place double sided tape or glue on the grey rectangle at the bottom and on top of each of the folded side tabs which will form the cd sleeve.
Turn over the front so that it is face down.
Line up the bottom of the back with the front and make sure the bottom and sides are lined up evenly before pressing back onto front.
Press firmly to seal tape or glue.
Fold the stand on the score line.
Using your thumb, begin to shape the curved score line down by slowly pressing down along the curve with your thumb as shown.
Use you finger to curl the bottom grey pieces slightly to a curved shape.
This will give the front some added strength when standing.
Slide in the cd. The center half circle tab that was folded down should click to hold the cd in place.
To remove, simply slide out. Tab should be pushed down again underneath when reinserting cd.
Open to stand the jukebox cd holder or lay flat to mail.
